Labelling and issue of Blood12. Labelling and issue of BloodBefore each donor unit is issued it shoud be properly labelled with patient’s details. The blood unit must be checked for any leaks, breaks changes in colour, odour or haemolysis before issue. The label should have the following information1. Recipient’s name, age, sex, registration no., ABO and Rh group. 2. Donor unit number, ABO and Rh group of donor, results of compatibility test and signature of the technical personnel performing the test. If blood is issued before compatibility tests are completed, label should clearly mention ‘un-crossmatched blood’. Compatibility report should also be issued alongwith the blood unit. Blood transfusion services should maintain records for each unit of blood issued. It should contain the following information: 1. Compatibility records 2. Blood group of donor 3. Blood group of patient 4. Component issued 5. Blood issued to whom (recipient) 6. Issued by whom (technical staff) 7. Date and time of issue 8. Records of transfusion reactions if reported.
